diabetes ketoacidosis จะเกิดขึ้นได้เมื่อไหร่

Diabetes ketoacidosis ในคนไข้เบาหวานจะเกิดขึ้นเมื่อไหร่

ในคนไข้เบาหวานที่มีน้ำตาลในเลือดสูงมากๆ ถ้าไม่อยากให้น้ำตาลในเลือดสูง คิดง่ายๆเราก็ไม่ต้องให้คนไข้ได้น้ำตาลเลย คือไม่ให้ทานของหวานเลย ไม่ให้ fluid ที่มีน้ำตาลเป็นส่วนประกอบ ถ้าเราทำเช่นนั้น คนไข้เบาหวานจะเกิดภาวะ diabetes ketoacidosis ได้ ทำไมจึงเป็นเช่นนั้น ทั้งๆที่คนไข้ก็มีน้ำตาลในเลือดสูงอยู่แล้ว ที่เป็นเช่นนั้นก็เพราะว่า น้ำตาลในเลือดของคนไข้เบาหวานนั้นร่างกายไม่สามารถนำไปใช้เป็นพลังงานได้ แต่ถ้าเป็นน้ำตาลที่ metabolite ขึ้นมาใหม่นั้น cell จะสามารถนำไปใช้ได้

เพราะฉะนั้นถ้าเราไม่ให้น้ำตาลเลย ก็จะเกิด Diabetes ketoacidosis ได้ แล้วเราให้น้ำตาลได้เท่าไหร่ละถึงจะน้อยที่สุดที่จะไม่เกิดภาวะนี้ ต้องมาเข้าใจกระบวนการ physiology ของร่างกายก่อน

ร่างกายเราแบ่ง cell ในการใช้น้ำตาลได้ประมาณ 3 ประเภท
1. glucose dependent คือ สมอง และ RBC แปลว่า สมองจะไม่ใช้พลังงานในรูปอื่นนอกจาก glucose
2. glucose prefer คือพวก ตับ ไต
3. glucose independent คือไม่มีน้ำตาลก็ไม่เป็นไร เช่น พวก cell กล้ามเนื้อ, nerve cell ใช้พวก lactic acid ได้

เพราะฉะนั้น ถ้าสมองมีน้ำตาลไปเลี้ยงไม่พอ ร่างกายก็จะสลายไขมันเพื่อไปสร้างน้ำตาลมาใช้ ทำให้เกิดของเสียที่เรียกว่า ketone ออกมา นำไปสู่ diabetes ketoacidosis

แล้วสมองต้องใช้น้ำตาลต่อวันขัั้นต่ำเท่าไหร่ถึงจะเพียงพอละ คำตอบอยู่ที่ glucose 120 g แล้วมันเท่าไหร่ละ glucose 120 g นะ คิดง่ายๆ coke หนึ่งกระป๋องมีน้ำตาลอยู่ประมาณ 200 g เพราะฉะนั้นถ้าคนไข้กินโค้กได้วันละกระป๋องก็ไม่ต้องกลัวภาวะ diabetes ketoacidosis ล่ะ (แต่ถ้ากินโค้กด้วย กินข้าวด้วย กินผลไม้หวานๆด้วย น้ำตาลก็ไม่ลงเลยละคราวนี้ ต้องดูหลายๆอย่างประกอบ นี้แค่ยกตัวอย่างให้ดู)

Regular insulin กับการควบคุมน้ำตาลในผู้ป่วยเบาหวานก่อนผ่าตัด

ในกรณีคนไข้ เบาหวานที่ต้องควบคุมระดับคนไข้เบาหวานก่อนผ่าตัด ส่วนใหญ่จะใช้ regular insulin ในการควบคุม

การให้ RI ให้ได้หลักๆสองทาง
1. IV ออกฤทธิ์ทันที peak ใน 30 นาที
2. subcutaneous ออกฤทธิ์ช้า peak ที่ 3 hr และมีผลต่อไปอีก 3 hr

โดย RI 1u จะสามารถลดน้ำตาลได้ 30-50 mg%
ถ้าคนไข้มี FBS > 200 จะ 1u จะลดได้ 50mg%
แต่ห้ามลด FBS > 100 mg% ใน 1 hr มิฉะนั้นจะเกิดภาวะสมองบวมจาก ภาวะที่ cell ได้น้ำตาลเข้าไปมากทำให้น้ำไหลตามน้ำตาลเข้าไปใน cell เกิด cell บวมขึ้น

ข้อควรรู้อีกอย่างคือ น้ำเกลือ 5%Dn/2 1 ขวด มีน้ำตาล 50 g ซึ่งต้องใช้ RI 10u ในการ metabolite ให้หมด ซึ่งก็คือ RI 1u metabolite glucose ได้ 5 g ความรู้นี้นำไปใช้ในการ titrated RI ผ่านทาง IV drip ได้

ข้อที่ควรนำมาพิจารณาเกี่ยวกับระดับน้ำตาลกับศัลยกรรม อีกอย่างหนึ่งคือ ระดับน้ำตาลหลังทำหัตถการเป็นเช่นไร เนื่องจากการทำ surgery จะทำให้เกิด trauma หลังทำ ทำให้ภาวะน้ำตาลสูงขึ้น แต่อีกทางหนึ่งถ้าคนไข้หลังทำหัตถการไม่สามารถกินได้ ระดับน้ำตาลในเลือดก็จะต่ำ ดังนั้นเราต้อง balance สองจุดนี้ให้ดี

สุดท้ายนี้ความรู้ด้านบนเป็นอัตรา RI ที่ลดน้ำตาลได้ในคนปกติ แต่ในคนเป็นเบาหวาน cell จะ response ต่อ RI ได้ไม่เหมือนคนปกติ ดังนั้นต้องทำการ titrated โดยปรับ dose ไปเรื่อยๆแล้วเจาะ Dtx เป็นระยะเผื่อประเมินว่าในคนไข้คนนี้ reponse ต่อ RI อย่างไร

staging of osteoradio necrosis (ORN)

osteoradionecrosis เราสามารถแบ่งออกเป็นทั้งหมด 3 stage โดย stage III ตามตัวหนังสือที่เขียนไว้ก็คือ ลักษณะของ ORN ร่วมกับมี fistula opening หรือมี pathologic fracture ของตัวกระดูกร่วมด้วย ซึ่งถ้าเราแบ่ง stage ของ ORN ตามลักษณะของมันนั้น เราจะยังเข้าไม่ถึงหัวใจของการแบ่ง ORN

อธิบายอีกทางก็คือ สมมุติเราพบ ORN ที่มี fistula openning แต่เราส่งไปทำ HBO แล้วอาการผู้ป่วย response ดีขึ้นจนหายดีโดยเราไม่ต้องทำ intervention ใดๆเลย เรายังจะให้เป็น ORN stage III หรือไม่

ดังนั้น จริงๆแล้วการแบ่ง stage ORN อย่างง่ายๆเลยก็คือ ถ้า ORN ที่ response ต่อ HBO 3o drive แล้วสามารถรักษาหายได้โดยไม่ต้องทำ local debridement ใด เราถือว่าเป็น stage I แต่ถ้าได้ HBO 30 drive อาการไม่ดีขึ้น ต้องทำ local debridement แล้วรักษาหายได้ เราถือว่าเป็น ORN stage II และสุดท้ายถ้ารักษาด้วยการ local debridement ไม่หาย ต้องใช้การรักษาที่ aggressive มากขึ้นไปอีก เราก็จะถือว่ามันเป็น ORN stage III

In maxillofacial surgery, the oculocardic reflex can be occur when we explore orbit or any operation that have to associated with orbit.

The oculocardiac reflex, also known as Aschner phenomenon, Aschner reflex, or Aschner-Dagnini reflex, is a decrease in pulse rate associated with traction applied to extraocular muscles and/or compression of the eyeball. The reflex is mediated by nerve connections between the trigeminal cranial nerve and the vagus nerve of the parasympathetic nervous system. The afferent tracts derive mainly from the ophthalmic division of the trigeminal nerve, although tracts from the maxillary and mandibular division have also been documented[1]. These afferents synapse with the visceral motor nucleus of the vagus nerve, located in the reticular formation of the brain stem. The efferent portion is carried by the vagus nerve from the cardiovascular center of the medulla to the heart, of which increased stimulation leads to decreased output of the sinoatrial node[2]. This reflex is especially sensitive in neonates and children, and must be monitored, usually by an anaesthesiologist, during paediatric ophthalmological surgery, particularly during strabismus correction surgery[3]. However, this reflex may also occur with adults. Bradycardia, junctional rhythm, asystole, and very rarely death [4], can be induced through this reflex.

Treatment/prophylaxis

Removal of the inciting stimulus is immediately indicated, and is essential for successful termination of this reflex. The surgeon, or practitioner, working on the eye should be asked to cease their activity and release the applied pressure or traction on the eyeball. This often results in the restoration of normal sinus rhythm of the heart. If not, the use of an anti-muscarinic acetylcholine (ACh) antagonist, such as atropine or glycopyrolate, will likely successfully treat the patient and permit continuation of the surgical procedure. In extreme cases, such as the development of asystole, aggressive cardiopulmonary resuscitation may be required.

fracture mandible and airway obstruction

Fracture mandible that will increase the risk of airway obstruction mostly appears in 2 form.

  1. Fracture bilateral symphysis of mandible
  2. Fracture bilateral condyle of mandible

First, the symphysis acted like free segment (coz fracture bilat.), suprahyoid muscle (ex. anterior belly of digastric) will pull this segment back, The oral cavity appeared smalller, tongue have no space to occupy. This sequence make patient difficult to breath.

Latter, the fracture of bilateral condyle of mandible, in this type when patient lie down the mandible will retrude. In the same sequence, tongue have no space to occupy and patient is hard to breath.

Thus the patients in these two type (maybe) can't lie in supine position, the best position is lie on one side or semi supine position to make sure the fracture fragment will not retrude posterior.
